I don't see how they can work with an individual, because Medicaid only works with agencies with supervising RN's and MD's, a lot of insurance and a continuing education program. Not to mention background checks.
You might contact your state Dept of Health & Human Services to see what is needed for on independent vendor to qualify to be paid for home / community based care under the state's Medicaid program. If you qualify for state (Medicaid) paid for then you should also qualify for Medicare.
Is any of this hospice? If so another thought is to see if you can perhaps work as a limited services contractor for a local hospice agency. Hospice is all Medicare paid.
(By sole practitioner, I mean a certified home health aide, working with a private client in their home, a client eligible to receive Medicare to pay for this kind of service....)
