I am an OT who works in home health. This would be something an OT would do for you. I would ask your family Dr. to make a referral to your local home health agency for an OT assessment.
My husband's doctor ordered a therapist visit to the home for this purpose. (Can't remember whether it was PT or OT.) When my son was in rehab after an accident they sent a therapist to his home to see how he could function in that environment before they released him. They made a few suggestions for improvement.
I would start by calling or emailing the clinic where the dementia is being treated and asking for an order for such an evaluation. It is very worthwhile!
