What agency in Oklahoma would you report problems in a Memory Care Center?

Many memory care facilities are private pay and considered assisted living facilities which are licensed differently than Medicare licensed nursing homes

Contact the local ombudsman - this contact should be clearly posted somewhere in the facility and they can direct you from there

The state health department if that is the agency that licenses the facility. If it is a Medicaid facility also contact them. Call the Area Agency on Aging they are an excellent resource and have ombudsmen on staff that woyld pay the facility and your mother a visit.
