Is there a bed in a facility in NY waiting for your mom? If not, utilize the resources at the current NH, the social worker, and get your mom into a new facility in NY. The social worker can help you find a suitable facility and can contact his/her counterpart there about your mom's need for a placement.

LBNY, please note if your cross State line, the Medicaid in Penn will not follow your Mom. Your Mom would need to apply for Medicaid in New York. It is recommended that you start that process ASAP.
