How do I activate my mother’s power of attorney as she has been deemed incompetent?

How does the POA read. Mine read when Mom was found incompetent. I did not need anything from the doctor. My POA was always excepted.
A "springing" POA will say that in the wording. Some members say its in effect at the time its signed.

What are the springing documents necessary to trigger POA? Letter from one or two doctors? You need those.
