Closer meaning the same State? If not and LO is on Medicaid transferring would be a problem. Medicaid doesn't go over state lines.
If in the same state you find the NH you want to transfer her to and work things out with that NH. They may help you with the other NH.

Talk to the receiving nursing home. They should help to arrange everything.
